Mission: To provide compassionate care by healing, comforting, and bringing light to those we serve.
Commonheart Home Health and Hospice is a service of Illuminus–a not-for-profit organization and a faith-based community open for all, sponsored by the Western District of the Moravian Church, which has a tradition of caring since 1969.
What We Do
We provide home health and hospice services in 32 counties across the southern half of Wisconsin.
