

Who We Are
Mission: To provide dental, mental health, and medical services for the low income uninsured and underinsured.
Church Health Services (CHS) is a Christian-based healthcare organization that provides dental, medical and mental health care programs and services for low income children and adults living in south central wisconsin. CHS also partners with local congregations to help them become centers for health and healing.
What We Do
Church Health Services, CHS, serves low income adults and children who reside in South Central Wisconsin.
- Medical Care Walk In Clinics are held throughout the month at different times and dates. Qualifying adults who are between the ages of 18 and 64, low income and have no insurance are welcome to come to these clinics.
- Mental Health Care Clinics and Services are available by appointment to qualifying adults, adolescents and children. We are a state certified mental health and AODA clinic. Services offered are counseling, alcohol and other addictions (AODA) counseling are available and affordable to families and individuals. Counseling sessions are held in our CHS offices in Beaver Dam. Children’s counseling is also available at a number of school districts in Dodge and Fond du Lac Counties.
- Dental Services are are available at our Beaver Dam Clinic by appointment and include comprehensive dental exams and treatment recommendations, dental cleanings are provided by dental hygienists. Some of our services include x-rays, fillings, extractions and root canals.
